Menopur: instructions for use and reviews

Menopur is a menopausal human gonadotropin drug belonging to the group of menotropins.

Release form and composition

Dosage form - lyophilisate for the preparation of solution for injection, which is a briquette of almost white or white color (in bottles of 2 ml, 5 bottles complete with 5 ampoules of solvent (1 ml each) in a cellular contour packaging, in a cardboard pack 1 or 2 packs and instructions for use of Menopur).

Active ingredients:

  • Luteinizing hormone – 75 IU (international units);
  • Follicle-stimulating hormone – 75 IU.

Additional components: sodium hydroxide, hydrochloric acid, polysorbate 20, lactose monohydrate.

Solvent (colorless transparent solution): isotonic sodium chloride solution.

Pharmacological properties

Pharmacodynamics

Menopur is a highly purified human menopausal gonadotropin (hMG) preparation. Belongs to the group of menotropins. The drug contains FSH (follicle-stimulating hormone) and LH (luteinizing hormone) in a 1:1 ratio. The drug is obtained from the urine of postmenopausal women.

Main effects of Menopur:

  • Women: promotes an increase in the level of estrogen in the blood and the growth and maturation of ovarian follicles, proliferation of the endometrium;
  • Men: increases the concentration of testosterone in the blood and stimulates spermatogenesis (due to the effect on the Sertoli cells of the seminiferous tubules).

Pharmacokinetics

The maximum plasma concentration of FSH in the blood is achieved 6–24 hours after intramuscular administration, then the concentration level gradually decreases. The half-life of menotropins is from 4 to 12 hours.

Indications for use

For women:

  • Infertility due to hypothalamic-pituitary disorders (the purpose of using the drug is to stimulate the growth of one dominant follicle);
  • Carrying out assisted reproductive technologies (the purpose of prescribing the drug is to stimulate the growth of multiple follicles and the onset of conception).

For men: stimulation of spermatogenesis in case of oligoasthenospermia or azoospermia that developed as a result of primary or secondary hypogonadotropic hypogonadism (in combination with human chorionic gonadotropin (HCG) preparations, for example, Choragon).

Contraindications

Absolute for patients of both sexes:

  • Hyperprolactinemia;
  • Tumors of the hypothalamic-pituitary region;
  • Diseases of the thyroid gland and adrenal glands;
  • Hypersensitivity to the components of Menopur or other menotropins (drugs containing follicle-stimulating and/or luteinizing hormone).

Additionally for women:

  • Pregnancy;
  • Uterine fibroids or abnormal development of the genital organs incompatible with pregnancy;
  • Vaginal bleeding of unknown etiology;
  • Primary ovarian failure;
  • Cyst formation or persistent ovarian enlargement not associated with polycystic ovary syndrome;
  • Cancer of the breast, ovaries, uterus;
  • Lactation period.

Additionally for men: prostate cancer or the presence of other androgen-dependent tumors.

Menopur, instructions for use: method and dosage

Menopur is administered subcutaneously (SC) or intramuscularly (IM). The lyophilisate is dissolved with the solvent supplied in the package immediately before injection.

Unless otherwise prescribed by your doctor, the dosage regimens described below are usually used.

For women:

  • Infertility caused by hypothalamic-pituitary disorders: the dose and duration of treatment are determined individually depending on ultrasound data, the level of estrogen in the blood and clinical observation. The initial daily dose is usually 75-150 IU. If the ovaries do not respond, the dose is gradually increased until an increase in estrogen levels in the blood or follicular growth is noted. Next, the selected dose is used until the estrogen concentration reaches the preovulatory level. If estrogen levels rapidly increase at the beginning of stimulation, the dose is reduced;
  • Ovulation induction: 1-2 days after the end of Menopur therapy, human chorionic gonadotropin is administered once at a dose of 5000-10,000 IU.

To stimulate spermatogenesis, men are administered 1000-3000 IU of hCG 3 times a week until the level of testosterone in the blood normalizes. Then Menopur is administered 3 times a week at a dose of 75-150 IU for several months.

Side effects

  • Gastrointestinal tract: sometimes – colicky pain, nausea, vomiting;
  • Allergic reactions: arthralgia; in rare cases - hypersensitivity reactions (including increased body temperature, skin rashes); in some cases, with long-term treatment - the formation of antibodies and, as a result, a decrease in the effectiveness of the drug;
  • Endocrine system: in men – gynecomastia; in women - a sharp increase in estrogen excretion in the urine, mastalgia, formation of ovarian cysts, moderate (uncomplicated) enlargement of the ovaries;
  • Local reactions: itching, swelling, redness at the injection site;
  • Other: decreased blood pressure, decreased amount of urine excreted by the kidneys, increased body weight, multiple pregnancy;
  • Ovarian hyperstimulation syndrome: early symptoms - weight gain, nausea, vomiting, severe pain in the lower abdomen, increased number of red blood cells in plasma, electrolyte imbalance, hypovolemia, ascites, hydrothorax, hemoperitoneum (presence of blood in the abdominal cavity), thromboembolic syndrome.

Overdose

No information available.

special instructions

Before prescribing Menopur, it is recommended to correct hemoconcentration, as well as appropriate treatment in the presence of concomitant diseases, such as tumors of the hypothalamic-pituitary region, hyperprolactinemia, hypothyroidism, and adrenal insufficiency.

The drug is usually ineffective in men with primary testicular failure, as evidenced by high levels of follicle-stimulating hormone.

Before starting to use Menopur for infertility in women, the condition of the ovaries should be assessed: conduct an ultrasound examination and determine the level of estradiol in the blood plasma. The same measures must be taken systematically throughout the course of treatment, daily or every other day.

It should be borne in mind that menopausal human gonadotropin preparations can cause ovarian hyperstimulation in women, which manifests itself with clinical signs after the administration of hCG preparations for the purpose of ovulation and is expressed in the form of the formation of large ovarian cysts. This process may be accompanied by thromboembolic syndrome, ascites, arterial hypotension and hydrothorax with oliguria.

When the first signs appear (abdominal pain and/or formations in the lower abdomen, palpable or detected by ultrasound), which may indicate ovarian hyperstimulation, Menopur should be discontinued immediately.

If ovarian hyperstimulation develops, hCG should not be administered for the purpose of ovulation.

Use during pregnancy and lactation

Menopur 75 IU is not prescribed during pregnancy/lactation.

Drug interactions

If necessary, Menopur can be used in combination with Choragon (hCG drug) to induce ovulation in women (after stimulating follicle growth) and stimulate spermatogenesis in men.

Shelf life – 2 years.

Infertility is one of the problems that has plagued women for more than a hundred years, but with the deterioration of the environment, the harmful effects of external factors and poor heredity, more and more patients are suffering from this problem.

But medicine also does not stand still, inventing more and more drugs for treatment.

One of them is the drug "Menopur", which is one of the ten most popular in the CIS countries.

pharmachologic effect

A drug such as Menopur can be classified as a gonadotropin that stimulates ovulation, although the main difference from other tablets is that it is based on human gonadotropin.

“Menopur” contains FSH and LH in equal proportions, since it is made from the urine of women during menopause.

Main Impact The drug is aimed at increasing estrogen levels in the blood, stimulating follicle growth and normalizing the endometrium.

As for men, they can be prescribed the drug "Menopur" in cases where they need to increase testosterone levels and stimulate spermatogenesis, increasing the chances of fertilization. Since then the effect is directly on the seminal canals.

Composition and release form

The drug "Menopur" is sold in the form of bottles, but you need to start the course only after consulting a gynecologist

The main active component will be menotropin in the amount of 75 IU.

But effectiveness is achieved due to other substances in the preparation: lactose monohydrate, polysorbate, hydroxide, hydrogen acid.

Since the drug is used together with a solvent, for better absorption it contains sodium chloride, acid and purified water for injection.

The drug "Menopur" is sold in the form of ampoules with solvents in packs of 5 and 10 pieces each.

Indications for use

The most common causes of infertility

The use of the drug "Menopur" is allowed only after visiting the hospital, taking tests and developing an individual course from a doctor.

Injections are usually used in several cases:

  • Infertility due to anovulation, polycystic ovary syndrome, when taking clomiphene does not produce results;
  • To control ovarian hyperstimulation, follicle stimulation during infertility treatment, artificial insemination;
  • Stimulation of spermatogenesis in men;
  • Treatment of hypoganodism.

In other cases, the drug should be used very carefully, only in some cases under the supervision of a doctor.

This could be seborrhea, cancer prevention, normalization of hormone levels. Then the methodology, duration of the course and dosage of injections of this medicine may differ.

Contraindications

There are also a number of contraindications in which taking the drug "Menopur" is prohibited or the course may differ or be shortened, and you should immediately inform your doctor about this:

One of the contraindications is an ovarian cyst, a hobby of size
      • Abnormal development of the genital organs;
      • Bleeding and vague vaginal discharge;
      • Oncology of the genital organs;
      • Ovarian failure;
      • Pregnancy;
      • Breastfeeding period;
      • Tumors in the hypothalamus and pituitary gland;
      • Intolerance to some components of the drug "Menopur".

If you have one of the contraindications, then you need to find an alternative treatment. The same applies to situations where problems began after taking the medicine. This may be preceded by a number of side effects.

Side effects

In the first days of taking Menopur, most patients may experience side effects that are associated with the body's reaction and stimulation of the follicles.

Single, rare, common and very common side effects occur:

      • Ovarian hyperstimulation;
      • Pain, burning sensation in the lower abdomen;
      • Pain during sexual intercourse;
      • Headache, migraine;
      • Nausea and vomiting;
      • Edema;
      • Hematomas, bruises at the injection site;
      • Gynecomastia.

There are also more dangerous manifestations after Menopur injections, which include torsion of the ovaries, the appearance of other symptoms, and worsening of existing ones.

This is what ovarian torsion looks like

If side effects occur in a severe form, for more than five days, you also need to go to the hospital.

Drug interactions

It is worth saying that no drug interaction studies have yet been conducted with the drug Menopur.

Although when combined with Clomiphene, you may encounter an increased reaction of the follicles.

You can learn more about the drug Clomiphene here.

If you take the drug "Menopur" with agonists, you will have to increase the dosage of the first drug in order to achieve the desired ovarian reaction and stimulate follicle growth.

Regarding combined use with alcohol, drugs for epilepsy and tuberculosis, no negative reactions were identified.

Menopur and alcohol

There are no clear contraindications when using Menopur together with alcohol, but it is better to avoid it to improve the results of treatment.

Since alcohol reduces the activity of sperm, reduces the activity of follicles, complicates fertilization and the passage of the zygote through the fallopian tube.

It is better to consult your doctor in advance about combining alcohol with this drug.

Instructions for use and dosage

Menopur is taken as an intravenous injection, but before this you need to prepare a solution from a bottle and water for injection in a syringe. Although doctors recommend doing this only before administering the drug Menopur.

Typically, the dosage and duration of the course are developed by the doctor, depending on the problem and special contraindications in patients.

The minimum dose of Menopura for the treatment of infertility in women is 15-100 IU, approximately two bottles per day. 48 hours after the first injection of Menopur, diagnostics are carried out to determine the quality of the follicles and the reaction of the ovaries. If it is absent, then the dosage is increased.

If it is necessary to induce ovulation, then take 5000-10000 IU two days after the start of the Menopur course.

Regarding the treatment of men and stimulation of spermatogenesis, they are administered 1000-3000 IU three times a week. After a few months, an examination is carried out and an additional 75-150 IU of Menopur injections are administered per day.

Cases of overdose with Menopur have not yet been established, but if the dose is increased without a doctor’s permission, the following symptoms may appear:

  • Nausea;
  • Vomit;
  • Ovarian enlargement;
  • Change in body weight;
  • Diarrhea;
  • Dyspnea.

Then you need to go to the hospital or see your doctor, although most symptoms of an overdose with Menopur go away on their own after a couple of weeks.

special instructions

Before starting a course of Menopur injections, it is necessary to undergo a full examination by a doctor, have your partner’s sperm tested, treat kidney or liver failure, and examine ovarian cysts or tumors for malignancy.

You need to understand that taking Menopur can stimulate multiple pregnancies naturally. Although miscarriages may occur in the early stages of pregnancy after fertilization.

Regarding oncology, taking Menopur injections does not affect this fact, although patients often encounter fetal pathologies.

The main danger is that the drug "Menopur" leads to ovarian hyperstimulation, then you need to consult a doctor, treat the symptoms and refuse injections of this drug.

Price for the drug

You need to buy Menopur injections separately from the solvent for injections, so for the first set of 10 pieces you need to pay about 11,000 rubles, and for a package of solvent you will have to pay almost 12,000 rubles. We are talking about injections of 75 IU.

If necessary, you can select substitute analogues with a more affordable cost. Merional, Humog, Bravel and Fostimon, although Luveris are no less effective.

They all have the same active ingredient, pharmacological action and indications for use. The price fluctuates around 5,000 rubles per bottle with a dosage of 75 IU.

The active ingredient in Menopur substitute preparations is gonadotropin or folitropin beta.

But before the appointment, you need to consult a doctor, get tested and start the course without interruption from Menopur injections.

Women

When diagnosed, due to disturbances in hypothalamic-pituitary system (to stimulate growth dominant follicle ), the dosage regimen is set individually. The optimal dosage and duration of treatment is selected based on Ultrasound of the ovaries , blood concentrations estrogen and further clinical picture. About maturation dominant follicle can be judged based on the increase in plasma levels estrogen .

The initial daily dosage of Menopur is 75-150 IU. In case of no response ovaries a gradual increase in Menopur doses is indicated until the plasma level increases estrogen or trackable growth follicles . In this case, the administration of the drug is continued in the dosages currently used until estrogens level corresponding preovulatory period . With a rapid increase in level estrogen observed at the beginning of stimulation, the dose of Menopur should be reduced. To activate ovulation , after 1-2 days after the last dose of Menopur, a single injection of 5000-10000 IU is performed human chorionic gonadotropin (Horagon ).

For men

For stimulation of spermatogenesis preliminary administration of 1000-3000 IU 3 times a week is indicated human chorionic gonadotropin person, up to normalization of plasma levels testosterone . During treatment infertility it is necessary to conduct a preliminary assessment of the condition ovaries . The criteria for such an assessment are: Ultrasound and determination of plasma level estradiol . Throughout the course of treatment, similar studies must be initiated daily or every other day.

A fairly large number of married couples suffer from infertility. Modern medicine has great potential in solving this problem, but patients must understand that the therapy will be long-term. Menopur is a drug that is also used in the treatment of infertility. In this article we will try to figure out what its mechanism of action is and what indications for use it has.

Release form and composition of the drug "Menopur"

The product is available in ampoules.

Composition: the main substance is follicle-stimulating and luteinizing hormone (LH) in a 1:1 ratio.

Additional components:

  • Lactose monohydrate.
  • Polysorbate 20.
  • Sodium hydroxide.
  • Hydrochloric acid.

Also included with the medicine is an ampoule with a solvent, which contains sodium chloride and diluted hydrochloric acid to create a normal pH level.

The drug is produced in a cardboard box, which contains 5 ampoules with medicine and 5 with solvent.

The drug "Menopur" contains specially purified human menopausal gonadotropin. It is obtained from the urine of the female half of humanity after they have passed menopause. When the drug “Menopur” is introduced into a woman’s body (reviews and the results of numerous studies confirm this), the production of estrogen and the maturation of follicles are stimulated. In addition, proliferative processes occur in the endometrium.

If you administer the drug to a man, the level of testosterone in his body increases. This drug also stimulates spermatogenesis due to its effect on the cells of the seminiferous tubules.

"Menopur": indications for use

Men are also advised to use Menopur. Reviews and treatment results confirm its effectiveness for azoospermia and oligoasthenospermia, which are caused by primary or secondary but only the drug must be combined with another drug of human chorionic gonadotropin - hCG.

"Menopur": the right method for women

Women are recommended to take a course of injections for infertility, which is caused by problems in the areas of the brain responsible for the production of female hormones. For each woman, the dosage is selected individually. The optimal amount of the drug and course of therapy are established only after all studies have been completed:

  • Ultrasound of the ovaries.
  • Determination of the level of female hormones in the patient’s blood.
  • All necessary tests.
  • Conversations with a doctor.

Start administering the drug "Menopur" (reviews and treatment results are evidence of this) with 1-2 bottles per day (75-150 IU). If the ovaries do not respond, the dose must be gradually increased until estrogen levels begin to increase along with the growth of the follicles. After this, the dosage is maintained at this level until the level of estrogen in the blood becomes the same as before ovulation. If estrogen levels rise too quickly, the dose is reduced.

In order to induce ovulation, 5,000-10,000 IU of hCG is administered once every other day, or at least two days after the last Menopur injection.

"Menopur" for the strong half of humanity

" Menopur" for men (reviews and results of use indicate this) are recommended to stimulate the process of sperm formation. The medicine is administered in the amount of 1,000-3,000 IU of hCG three times a week until testosterone levels in the blood normalize.

After this, the drug should also enter the male body three times a week, but in the amount of 1-2 bottles (75-150 IU) for several months.

Contraindications for use

"Menopur" is an excellent drug, but it has its own contraindications for use, which every patient should know about before starting therapy in order to inform their doctor about them and, together with him, select a replacement or the correct dose. It is not recommended to take the medicine for the following pathologies:

  • Problems with the adrenal glands and thyroid gland.
  • Tumor neoplasms in the hypothalamic-pituitary region.
  • Hyperprolactinemia.
  • An increase in the size of the ovaries or the presence of a cyst that is not associated with the symptom of polycystic ovaries.
  • Bleeding from the vagina of unknown nature.
  • Anomalies in the development of the genital organs.
  • Uterine fibroids that are not compatible with bearing a child.

  • Cancerous tumors in the uterus, ovaries, mammary glands.
  • Stage 1 ovarian failure.
  • Pregnancy and breastfeeding.

"Menopur" to stimulate superovulation

Before starting IVF, women are prescribed a drug that will help stimulate superovulation. There are a lot of such drugs: Diferelin, Cetrotide, Puregon and many others.

"Menopur" for men in the IVF protocol is considered one of the effective drugs that has shown good results after the procedure. The doctor will help you choose the right protocol in each individual case. The specialist will take into account all the research conducted and select the correct course and dosage, which will help increase the positive result of the IVF procedure several times.

"Menopur" and its side effects

After using the drug "Menopur", reviews and results of IVF in most cases are positive, this made many families happy.

However, you need to remember that it also has side effects:

  1. From the digestive system: vomiting, nausea and flatulence.
  2. From the endocrine system: increased excretion of estrogen in the urine, pain in the lower abdomen. Men may experience gynecomastia.
  3. From the metabolic processes of the body: blood thickening, water and electrolyte disturbances, ascites and hydrothorax.
  4. In addition, during the period of use of the drug, many patients observed manifestations of an allergic reaction, as well as swelling and itching at the injection site.

"Menopur": special instructions for use

Before starting treatment, every woman and man should know that there are many restrictions and caveats. The instructions for use included with the medication provide detailed information about all this. The price (reviews note it as quite high, but not exorbitant) varies between 1,500 rubles per injection. However, the drug is effective and has allowed many families to experience parental happiness.

Before use, it is necessary to exclude ovarian exhaustion or resistance syndrome, extragenital endocrinopathies.

Also, every woman who undergoes stimulation with Menopur (patient reviews especially note this fact) should know that the result can be a multiple pregnancy.

If the patient shows signs of ovarian hyperstimulation, treatment should be stopped immediately. This can be indicated by pain in the lower abdomen, as well as by ultrasound. During treatment, it is necessary to monitor hormone levels every day and do an ultrasound of the developing follicles.

If pregnancy has already occurred, the symptoms of excessive hyperstimulation can increase significantly and be observed in the patient for quite a long time, which can serve as a serious threat to her life.

During therapy in men, with a high concentration of FSH in the blood, menotropins will not show positive results.

"Menopur": reviews

Currently, many women cannot become pregnant naturally, which is why they agree to the IVF procedure, which gives them a chance to have a child. But it doesn’t give 100% results the first time, so you have to carry out the procedure several times. But not every woman has such an opportunity, since it is not cheap. In order to give women the opportunity to become pregnant the first time as a result of IVF, the drug Menopur 300 IU was developed. Reviews and results of therapy are overwhelmingly positive. The medication increases the chances of getting pregnant several times.

The drug gives very good results after therapy, patient reviews are only positive, the only thing that scares many of them is multiple pregnancies, which can occur after a course of therapy with Menopur. There are also many contraindications and side effects, but if you are always under the supervision of a doctor who will monitor the patient’s condition and control the dose, then all unwanted manifestations will not be terrible.

In conclusion, I would like to say: a woman’s desire to have a child is so great that she is ready to do anything just to know the happiness of motherhood. Therefore, therapy with Menopur with all its side effects is not scary for her. But it is worth remembering that a rash decision and uncontrolled use of medicine can lead to very serious and irreparable consequences. Only the doctor must go through this path together with the woman and do everything possible to get a positive result.
