Registration of individual entrepreneurship involves filling out a special form. In 2015, the form has not changed. Let's look at an example of filling out form P21001

Title page P21001

Fill in the last name, first name, patronymic (in block letters, put dashes in empty cells).

Filling out an application for registration of individual entrepreneurs in 2015

Any citizen can register himself as an individual entrepreneur. To do this, he needs to submit several documents, including an application that is filled out according to the approved form - p21001. Find out how to arrange it correctly in 2019 and what you need to pay attention to in this article.

The main purpose of the document is to declare your intention to become an individual entrepreneur. Existing form has been in effect since July 2013 and is significantly simplified compared to the previous version. As before, the application consists of 4 pages, the contents of which are described in the table.

  • full name (patronymic if available), as indicated in the passport; if the person is a foreigner, then the data is filled out in Latin;
  • field mark;
  • date of birth and place of birth using horizontal abbreviation;
  • citizenship (in the case of non-residents of the Russian Federation, the appropriate mark is placed and the country of origin is indicated in the form of a digital code).

Requirements for filling

Since the form p21001 is intended to be read in in electronic format with subsequent processing of data by a computer, then several requirements are provided for its design:


  • personally;
  • through a proxy acting on the basis of a notarized power of attorney;
  • by post.

The application can be downloaded and printed, and then filled out handwritten with a black gel pen. IN tax office You must send a registered letter and then wait for the notification to be received. The letter itself must also include a description of the attachment (indicate the name of the document in the shipment, its type (original) and quantity - 1 piece).

Step-by-step instructions for filling out

IN general view instructions for filling out form p21001 look like this:

  1. All characters are written big signs(letters, numbers).
  2. The principle applies: one field (square) for only one character).
  3. Any dates must be filled in in numeric format without the abbreviation “g.” or the words "year". For example, if you need to enter the date February 1, 2012, you should do it like this.
  4. For simple or decimals special two fields are filled in, which are separated by the corresponding characters - slash / or comma “,”. In this case, it is necessary to fill in the fields closest to each other, as shown in the figure.
  5. If a certain characteristic has a monetary value - for example, the size of a share in the authorized capital, the nominal value of a share, then the value is written down to the nearest kopeck according to the same principle. If there are no kopecks, then the fields closest to the edge are still filled in, but no marks are placed in the field for tenths (i.e., zeros do not need to be written).
  6. For activity codes according to OKVED, it is necessary to use three fields at once, each of which is separated by a dot. The designation of the codes must correspond exactly to the generally accepted system, down to the sign.
  7. All other digital indicators (for example, date of birth) are written in the usual format, starting from the leftmost square - i.e. exactly the same as in other documents.
  8. When specifying the series and number of a passport (or other identity document), you must write all numbers separated by spaces: 2 digits of the series, a space, 2 digits of the series, a space and digits of the series in a row, as shown in the figure.
  9. When writing a contact number, you should also use each square for its own symbol, and brackets, + and other symbols are also filled in in a separate field. Landline and mobile numbers are entered in the internationally accepted format as shown below.
  10. When filling out fields with text (letter) data, you must start, as usual, from the leftmost square and provide each character separate field. If the text consists of several words, numbers and/or characters at once, then they are written together, without spaces. However, if this contradicts the rules of the Russian language, then spaces are left. A space means one square.
  11. If there is not enough space on one line, it is filled completely, and the next line continues, starting from the nearest square on the left side. In this case, no hyphens “-” are placed. For example, you need to write the name of the company, which is officially called public Joint-Stock Company"RomashkaPlus-7". Then all existing spaces between words are preserved in accordance with the rules of the Russian language. And the company name itself is written exactly as it is indicated, for example:
  12. If a word ends on the first line along with the end of this line, and the next word is written separated by a space, then on the second line it must begin from the second square. That is, all spaces are counted as an empty square in any case (regardless of where this space falls).
  13. The form assumes the use of generally accepted abbreviations, but without periods. However, initials are abbreviated along with periods, as is customary in most cases. For example, if the official name of the street is written as “street named after V.I. Lenin", in the application you should write "UL IM V.I. LENIN".
  14. All fields that have not been filled in (for example, fields with OKVED codes) simply remain empty - no dashes or other designations should be entered.

The upper right corner of the form contains information about the authority that approved the application form. You can print the document either with or without this information - in any case, the application will be accepted for processing provided that other registration conditions are met.

Application procedure

Any Russian citizen who is over 18 years old and has full legal capacity can register as an individual entrepreneur. Package of documents in in this case small:

  • application in form p21001, considered earlier;
  • original and copy of 2 pages (main and registration address) of your passport;
  • receipt confirming payment state duty– at the moment it is 800 rubles.

Having collected this package of documents, you need to do the following:

  1. Find out which tax office you need to contact. Apply in accordance with the official residence address (registration). If a citizen is in another region, he should obtain a certificate at his place of residence and contact the appropriate tax office. You can find out about your tax office address on the official website of the Federal Tax Service.

Fill out the address, indicate your status ( individual).

And the Federal Tax Service code is selected in accordance with the official code of the region - for example, 17 for the Republic of Tyva, 55 for the Omsk region, etc.

  1. After this, the documents are sent in person, by mail, through a representative (through State Services - if available technical feasibility). At the same time, along with the Federal Tax Service, you can also contact the MFC - centers for providing public services to citizens.
  2. The waiting period begins on the working day following the day of the visit to the authority (or notification of delivery of a registered letter). The application is reviewed within 3 working days, the response is sent to the specified address.

Registration refusal: what to do next

Any citizen of Russia, non-resident and even a stateless person has the right to register an individual entrepreneur. Therefore, refusal can only be due to formal reasons:

  1. The documents were not submitted in full.
  2. The documents were submitted to the wrong authority.
  3. Form p21001 was filled out with errors, typos, inaccuracies and/or blots.
  4. If the person is already registered as an individual entrepreneur.
  5. If a person was already an individual entrepreneur, but by a court decision he was declared bankrupt, and 1 year has not yet passed from the date of this decision.

In any case, the Federal Tax Service must send a written response confirming the acceptance of a positive or negative decision with justification for their actions.

Thus, filling out form p21001 according to the sample is quite simple - the main thing is not to miss a single detail and avoid mistakes, corrections and blots.

General requirements for filling out registration forms

If the Requirements are not met, the application for registration of an entrepreneur may not be accepted, so you need to pay attention to the following points:

  • can be filled out either by hand or on a computer, but both filling methods cannot be combined;
  • only capital letters are used printed letters black color;
  • if a completed application for registration of an individual entrepreneur is drawn up on a computer, then only Courier New font with a height of 18 points is allowed;
  • each page of an application for registration of a person as individual entrepreneur printed on a separate sheet;
  • the document should not contain blots, errors, or corrections;
  • The completed pages of the form are submitted to the tax office, and they are not stapled, but only secured with a paper clip.

These requirements are explained by the fact that registration forms are designed for automatic text recognition, and any shortcomings may prevent this from happening. Be careful when filling out the form to register a person as an entrepreneur, because there is a high risk of refusal due to a carelessly executed document.

Special requirements for filling out form P21001

But in addition to these general requirements, the order of the Federal Tax Service also approved special rules for processing application R21001. To avoid confusion and avoid mistakes, let’s look at a sample of filling out an application for individual entrepreneur registration page by page.

First page

Form P21001 page 1

Here you need to provide information about the individual who is registering as an individual entrepreneur: full name, individual tax number (TIN), gender, date and place of birth, citizenship.

In paragraph 1.1, all applicants indicate their full name in Russian: Russians, foreign citizens and stateless persons. Additionally, foreigners and stateless persons fill out paragraph 1.2, i.e. your name in Latin.

Most Russian citizens have a TIN, so if you have one, you need to enter your tax number in the next line. If you do not remember the TIN or do not know whether you received it, you can clarify this information through the Federal Tax Service service. But even if you don’t have a TIN, no problem, you will be assigned one during the process of registering an individual entrepreneur. For now, just leave the field blank.

Next, select your gender and proceed to entering information about the date and place of birth. This information must be entered exactly as indicated in your passport. All that remains is to choose citizenship (in clause 5.1, foreigners also indicate the country code according to the All-Russian Classifier of Countries of the World OK-025-2001), and with this, filling out the first page is completed.

Second page

Form P21001 page 2

The second page of the application for registration of individual entrepreneurs in form P21001 is intended for entering information about the place of residence and passport data. When indicating the residential address of an individual, the following points must be taken into account:

  1. Abbreviations of elements of the address of addressable objects must comply with Appendix No. 2 “Names of addressable objects” to the order of the Federal Tax Service of Russia dated January 25, 2012 No. ММВ-7-6/25@).
  2. If the entrepreneur lives in Moscow or St. Petersburg, then paragraphs 6.3-6.5 are not completed.
  3. The residence address for Russians is entered on the basis of registration in the passport, and in its absence - on the basis of a temporary registration certificate.
  4. Information about the address of residence (stay) in the Russian Federation for foreigners and stateless persons is indicated in accordance with the residence permit or temporary residence permit.

In clause 7.1 you must enter the code of your identification document. The full codes of these documents can be found in Appendix No. 3 to the order, and here we present the most popular codes:

  • “21” - Passport of a Russian citizen;
  • “12” - Residence permit in the Russian Federation;
  • “15” - Temporary residence permit in the Russian Federation.

The rest of the document data must fully correspond to the original.

Third page

Form P21001 page 3

This page is filled out only by foreign citizens and stateless persons. The Russians skip it and move on to the next page. That is, the completed form P21001 will have 5 pages when registering an individual entrepreneur by foreigners and stateless persons and 4 pages if the entrepreneur is a Russian citizen.

Paragraph 8 indicates the data of the document that confirms the right of an individual who does not have Russian citizenship to reside in the territory of Russia:

  • registration number of residence permit or permanent residence;
  • date of issue;
  • name of the FMS department;
  • validity period of the document.

Fourth page

Form P21001 sheet A

This page has its own name - sheet “A”. Species codes are entered into it entrepreneurial activity according to , and there must be at least four characters in the code, although the Classifier has two- and three-digit codes.

The number of codes for entering into the P21001 form is not limited, but only 57 codes can fit on one sheet “A”. If one sheet is not enough for you, then you can additionally fill out a new sheet, but the main sheet is entered once on the first sheet.

Fifth page

This page is called sheet “B”, and in it the applicant confirms the accuracy of the information and its compliance with the original documents.

Please note that in section 1, the full name of the entrepreneur must be filled out in person, even if the entire application is completed on a computer. Contact information requires a phone number, and email address indicate only if an application for registration of individual entrepreneurs is submitted electronically.

The applicant’s signature is not affixed in advance; signing can only be done in the presence of a tax inspector or notary.

XIV. Requirements for completing the Application for state registration of an individual as an individual entrepreneur (form No. P21001)

14.1. An application for state registration of an individual as an individual entrepreneur is drawn up if accepted by the citizen Russian Federation, as well as a foreign citizen or stateless person legally staying on the territory of the Russian Federation, decisions to carry out entrepreneurial activities without education legal entity.

14.2. In Section 1 “Last name, first name, patronymic of an individual,” paragraph 1.1 is filled out both in relation to a citizen of the Russian Federation and in relation to a foreign citizen or stateless person. In this case, in relation to a foreign citizen or stateless person, the section is filled out in accordance with the residence permit or temporary residence permit.

Clause 1.2 is filled out in addition to clause 1.1 by a foreign citizen or stateless person if the relevant information is available in the identity document in accordance with the legislation of the Russian Federation.

14.3. Section 2 “TIN” is filled out taking into account the provisions of paragraph 2.9.2 of these Requirements.

14.4. In section 3 “Gender”, in the field consisting of one familiar place, the corresponding digital value is entered.

14.5. Section 4 “Birth Information” indicates the date and place of birth of an individual.

14.6. In section 5 “Citizenship”, in the field consisting of one familiar place, the corresponding digital value is entered.

If the value is 2, clause 5.1 is filled in, which indicates digital code countries according to the All-Russian Classifier of Countries of the World OK-025-2001.

14.7. The section “For official marks of the registering authority” is filled out taking into account the provisions of paragraph 2.4 of these Requirements.

14.8. Section 6 “Address of residence (stay) in the Russian Federation” is filled out taking into account the provisions of paragraph 2.9.6 of these Requirements.

14.9. Section 7 “Identity document details” is filled out taking into account the provisions of subclause 2.9.5 of these Requirements.

14.10. Section 8 “Data of the document confirming the right of a foreign citizen or stateless person to reside temporarily or permanently on the territory of the Russian Federation” indicates the details of the residence permit or temporary residence permit.

In clause 8.1, in the field consisting of one familiarity, the corresponding digital value is entered.

14.11. Sheet A “Information on codes according to the All-Russian Classifier of Economic Activities” is filled out taking into account the provisions of paragraph 2.16 of these Requirements.

14.12. Sheet B is filled out taking into account the following.

14.12.1. In section 1, an individual registered as an individual entrepreneur, in the appropriate line, personally indicates his last name, first name, patronymic (if any) in Russian.

In the field, consisting of one acquaintance, the corresponding digital value of the method of issuing (sending) documents confirming the entry into the Unified State Register individual entrepreneurs, or decisions to refuse state registration.

In the appropriate field, indicate the telephone number by which you can contact the applicant, and if documents are sent to the registration authority using information and telecommunication networks common use, including the Internet, including a single portal of government and municipal services, the address is also indicated Email.

If an application is submitted to the registration authority directly by an individual registered as an individual entrepreneur, the specified individual (applicant), in the presence of an official of the registration authority who accepts documents, puts his signature in the appropriate line.

Otherwise, the documents are submitted to the registration authority, the authenticity of the applicant’s signature must be certified by a notary.

14.12.2. Section 2 “The application is submitted to the registration authority directly by the applicant and signed by him in the presence of an official of the registration authority. 14.12.3. Section 3 “Information about the person who certified the authenticity of the applicant’s signature in a notarial manner” is filled out taking into account the provisions of subclause 2.20.6 of these Requirements.

Advantages of doing business as an individual entrepreneur

Entrepreneurship in the form of an individual entrepreneur has significant advantages compared to doing business through a company. Main advantages:

  • Registering an individual entrepreneur is easier and cheaper than registering a company. The fee for registering an individual entrepreneur is 800 rubles, for registering a company - 4,000 rubles. If you turn to registrars for help when registering a business, then registering an individual entrepreneur will cost less than registering a company;
  • administrative fines for entrepreneurs are often lower than for companies;
  • entrepreneurs can do without registration cash orders for cash transactions and may not conduct cash book;
  • entrepreneurs are not required to keep accounting records;
  • only entrepreneurs can apply the patent tax regime;
  • entrepreneurs who have chosen a simplified or patent tax regime may be subject to tax holidays for a period of no more than 2 years according to certain types activities (scientific, industrial, social, consumer services).

Entrepreneur registration procedure

This is not a very complicated procedure. It is simpler and less expensive than creating and registering a company. To register an individual entrepreneur you need:

  1. Pay the state fee.
  2. Fill out the application (form 21001 approved by the Order of the Federal Tax Service dated January 25, 2012).
  3. Submit an application and document on payment of the duty to the tax authority at your place of residence.

That's all that is required for registration. If the documents are in order, within 3 days the individual entrepreneur will be registered, and an entry about this will be made in the register of entrepreneurs (USRIP). The entrepreneur will be given documents confirming registration, after which he can carry out activities.

If an entrepreneur wants to apply any special regime, then the tax authority will need to submit an application for the application of the selected special tax regime. An application for the simplified tax system is usually submitted along with documents for registration of an individual entrepreneur, but this can be done within 1 month from the date of registration.

As you can see, there is nothing complicated in this procedure. The most difficult thing is filling out the application, but even here everything is actually simple.

Form 21001 is filled out (a sample for the 2017 individual entrepreneur is provided below) in accordance with the requirements set out in the specified order of the Federal Tax Service. It can be written by hand or printed on a computer. You don’t have to have your signature certified by a notary if you sign the application when submitting documents to the tax office.

The hardest part about filling out the form is right choice OKVED codes. The application shall indicate OKVED codes the types of activities that the entrepreneur plans to carry out. At a minimum, the application must indicate one OKVED code. Maximum amount codes are unlimited.

If there are several types of activities, then one of the codes is indicated as the main one. They need to indicate the type of activity for which the largest amount of income is planned.
